3. Confirmation: The Disability Resource Center will confirm receipt of your documents and will contact you to approve your enrollment or request additional documents if needed.
Examples of acceptable disability verification documents include but are not limited to:
- Statements or letters on a physician's/medical professional's letterhead stationary.
- Statements, records or letters from a Federal Government agency that issues or provides disability benefits.
- Statements, records or letters from a State Vocational Rehabilitation Agency counselor.
- Certification from a private Vocational Rehabilitation or other Counselor that issues or provides disability benefits
Documents must be dated within 1 year.
Letters from a physician must state your condition qualifies under the American’s with Disabilities Act.
